Replacement certificates

Membership and fellowship certificates are issued to members, but remain the property of ACCA. These must be returned when membership ceases. Replacement certificates may be obtained for a fee of £31. An application form for a replacement certificate can be downloaded. Where a certificate has been lost, stolen or destroyed, the application for a replacement should state exactly what happened to the previous certificate. Replacement certificates required as a result of a change of name due to marriage or deed poll are issued free of charge.
